A very light nasal pillows mask (about 55 g) with a single self inflating AirPillow seal that fits into and around the nostrils. It comes with a choice of adjustable or Stretchwise headgear.

SizesAirPillow seal: single size (one spare seal code, 400HC125), Headgear: adjustable or Stretchwise (the US kit 400HC328 includes both)
Works withAll standard CPAP, APAP and BiPAP machines with a 22 mm tube connection (F&P lists it as compatible with all home PAP devices)

Usually suits

  • Nose breathers who want the smallest mask
  • People with beards or moustaches
  • People who feel claustrophobic in larger masks

Design

  • Self inflating AirPillow seal that seals into and around the nostrils
  • Two headgear options: adjustable and Stretchwise
  • Low profile frame and lightweight flexible tube
  • Q Cover elbow cover and diffuser to reduce noise and draft
  • Weight about 1.94 oz (55 g); operating pressure range 4 to 25 cm H2O

Strengths and trade-offs

  • Minimal contact: nothing on the bridge of the nose or around the mouth
  • No seal sizing needed
  • No magnets
  • Pillows direct air into the nostrils, which some people find drying or uncomfortable at higher pressures
  • Rated to 25 cm H2O
  • Mouth breathers need a chinstrap or a different mask type
  • One seal size can be a poor fit for very small or very large nostrils

Finding your size

There is no seal sizing gauge: the Pilairo Q uses one AirPillow seal that inflates with CPAP pressure to form a double seal into and around the nose. Fit is set by the headgear. The adjustable headgear covers a wider range of head sizes; the Stretchwise band adjusts itself. F&P's myMask app does not size pillow masks.

How to fit it

  1. Open the seal

Hold the silicone seal away from your nose and use your fingers to carefully spread the soft side wings.

  2. Insert

Guide the two openings of the seal into your nostrils.

  3. Place the headgear

Hold the frame with one hand and pull the headgear bottom strap down over the back of your head until it is secure. Do not let go of the mask until it is positioned on your face.

  4. Fix leaks with air on

With the machine running, if it leaks, pull the seal away from your face while the headgear stays on so the seal can inflate with air, then place it back so the two openings fit securely into your nose.

Common problems and fixes

Whistling or hissing at the nostrils

Pull the seal forward so it fills with air, then let it settle back into place. Do not push it harder into the nose.

Sore nostrils

Check the headgear is not pulling the seal upward, and use a water based nasal moisturizer if your clinician agrees. Persistent soreness or bleeding is a reason to talk to your provider.

Dry nose

Ask about increasing humidity in your machine's comfort settings, and consider saline spray. If your mouth falls open, add a chinstrap.

Cleaning and care

F&P instructions: after each use, wash the mask excluding the headgear in soap dissolved in lukewarm water, no soaking over 10 minutes, rinse thoroughly and dry out of direct sunlight. After 7 days of use, hand wash the headgear the same way. Do not use products containing alcohol, antibacterial agents, antiseptic, bleach, chlorine or moisturizer, do not use a dishwasher, and do not use ozone or UV "CPAP cleaners" (the FDA warned about these in 2020).

Supplies that can help

CPAP chinstrap

A soft strap that supports the jaw to help keep the mouth closed, used mainly with nasal and nasal pillow masks to reduce mouth leaks.

Saline nasal spray

Salt water spray that moistens and clears the nose, a simple aid for the congestion and dryness some people get on CPAP.

Nasal moisturizer gel

A water-based or saline gel applied inside or around the nostrils to soothe dryness and soreness from CPAP airflow.

CPAP mask wipes

Pre-moistened wipes for a quick daily wipe of the mask cushion, mostly to remove skin oils between proper washes.

Frequently asked questions

What size is the Pilairo Q nasal pillow?

It has a single AirPillow seal (spare part 400HC125) rather than small, medium and large pillows. The seal inflates with your therapy pressure to shape itself into and around the nostrils.

Which Pilairo Q headgear should I use, adjustable or Stretchwise?

The US headgear kit includes both. Stretchwise is a single elastic band that adjusts itself, and the adjustable headgear lets you set tension and fits a wider range of head sizes.

Is the Pilairo Q still made?

As of October 2026 Fisher & Paykel still lists the Pilairo Q and its spare parts on its US mask page. Availability at individual suppliers varies.
