React Health (formerly 3B Medical) · Under the nose full face mask (hybrid, minimal contact)
React Health Rio II Full Face Mask
A minimal contact full face mask from React Health. The cushion covers your mouth and seals under your nose instead of over the bridge, so it keeps pressure off the nose and leaves your view clear for glasses. It uses easy release clips, not magnets.
Usually suits
- Mouth breathers who get sore or red on the bridge of the nose
- People who feel claustrophobic in a traditional full face mask
- People who read or watch TV in bed with glasses on
- People who need a magnet-free full face mask
Design
- Under-the-nose full face cushion that leaves the bridge of the nose free
- Does not block your vision or get in the way of glasses, per React Health
- Circular dispersion vent, rated by React Health as low as 17 dB(A)
- Modular frame compatible with all cushion sizes (S, M, L)
- Adjustable headgear
- Easy release clips rather than magnets
- Built to come apart easily for cleaning
Strengths and trade-offs
- Covers the mouth, so it works for mouth breathers
- No contact on the nose bridge, which may reduce feeling closed in
- You can wear glasses or read with it on
- Magnet-free clips, an option where a mask with magnets is contraindicated
- React Health's public pages do not list a pressure range for this mask; check the instructions in the box
- Under-nose full face cushions can be harder to seal for some nose shapes than over-the-nose designs
- Fewer published specifications than older React Health masks
Finding your size
Cushions come in Small, Medium and Large, and the modular frame takes all three, so changing size only means changing the cushion. React Health has not published a sizing template for this mask on its product page, so the all size kit is the practical way to compare sizes: try each one lying down with your machine running at your usual pressure.
How to fit it
Check the parts
Make sure the cushion is fully seated on the frame and the vent is clear.
Place the cushion
Set the cushion over your mouth with the top resting under your nose, so your nostrils sit over the nasal openings and nothing rests on the bridge of your nose.
Bring the headgear back and clip it
Hold the cushion in place, pull the headgear over your head, then fasten the lower clips.
Adjust at pressure
Lie down in your usual sleep position, start your machine and adjust the straps evenly until the cushion seals. Snug, not tight: a full face cushion that is pulled too hard often leaks more.
Common problems and fixes
Leak under the nose or at the corners of the mouth
Reseat the cushion with the machine running and even out the top and bottom straps. If the leak continues, try another cushion size; the frame takes all three.
Air blowing toward the eyes
The top of the cushion has probably moved off its spot under the nose. Refit it so it sits under the nose.
Red marks on the chin or cheeks
Loosen the lower straps. Strap covers can reduce rubbing. Tell your provider about any skin breakdown.
Bloating or swallowing air
Tell your prescribing clinician, especially if it does not go away. Do not change your pressure yourself.
Cleaning and care
After each use, take the mask apart, wash the parts (not the headgear) by gently rubbing them for up to 10 minutes in warm water (about 86 F) with mild, unscented dish soap, clean the vent with a soft brush, rinse with drinking quality water and air dry out of direct sunlight.
Once a week, hand wash the headgear the same way, rinse and air dry. React Health says not to use anything with bleach, chlorine, alcohol, aromatics, moisturizers, antibacterial agents or scented oils, and to keep all parts out of direct sunlight and heat.
How often to replace parts
- Mask
- 1 every 3 months (A7030)
- Cushion
- Up to 1 per month (Medicare A7031), typical guidance; your plan may differ
- Headgear
- 1 every 6 months (A7035)
Supplies that can help
Soft fabric liners that sit between your skin and the mask cushion to reduce skin irritation, red marks and the noisy leaks some people get from …
Soft fabric sleeves that wrap mask headgear straps to stop red lines, rubbing and hair tangles.
Pre-moistened wipes for a quick daily wipe of the mask cushion, mostly to remove skin oils between proper washes.
Plain, mild dish soap is the cleaning agent most CPAP makers and the FDA point to for masks, tubing and humidifier chambers.

Frequently asked questions
Does the Rio II Full Face Mask have magnets?
No. React Health's brochure says the mask uses easy release clips, not magnets, and that these clips are common to all React Health masks.
Can I wear glasses with the Rio II Full Face Mask?
React Health says the mask does not interfere with vision or glasses, because the cushion sits under the nose instead of over the bridge.
What sizes does the Rio II Full Face come in?
Small, Medium and Large cushions on one modular frame, plus an all size kit (RFF1000). Replacement cushions and headgear are sold separately.
How is the Rio II Full Face different from the Rio II Nasal Mask?
Both use an under-the-nose cushion, but the full face version also covers your mouth, so it suits people who breathe through their mouth at night. The nasal mask covers only the nose.
