React Health (formerly 3B Medical) · Nasal mask
React Health Rio II Nasal Mask
React Health's newer nasal mask, separate from the Rio II pillows mask. It has an under-the-nose cushion that keeps pressure off the bridge of the nose, a 360 degree hose swivel, a circular vent React Health rates as low as 20 dB, and adjustable headgear.
Usually suits
- Nose breathers who get sore on the bridge of the nose
- People who need a magnet-free mask
- Active sleepers
Design
- Under-the-nose cushion, nothing on the bridge of the nose
- 360 degree hose swivel
- Circular dispersion vent rated as low as 20 dB
- Modular frame compatible with all three cushion sizes
- Adjustable headgear
- Hypoallergenic materials, per React Health
- Easy release clips rather than magnets, which React Health says are common to all its masks
Strengths and trade-offs
- No pressure on the nose bridge
- Magnet-free clips
- Swivel helps the hose follow you when you turn
- Same frame takes all cushion sizes, so a size change is just a cushion
- Nasal only; mouth breathers may need a chinstrap or a full face mask
- React Health's public pages do not list a pressure range for this mask; check the instructions in the box
- Newer mask with fewer published specifications than the pillows version
Finding your size
Cushions come in S, M and L, and the modular frame takes any of the three. An all size kit is listed. React Health has not published a sizing template specific to this mask (the fitting guide linked from its page is the pillows template), so the all size kit is the practical way to compare sizes with the machine running.
How to fit it
Check the parts
Make sure the cushion is pushed fully onto the frame and the vent is clear.
Place the cushion
Set the cushion under your nose so your nostrils sit over the opening, with nothing resting on the bridge of your nose.
Bring the headgear back
Hold the cushion in place and pull the headgear over your head, then fasten the clips.
Adjust evenly
Start your machine and adjust the straps evenly until the cushion seals comfortably. Snug, not tight.
Common problems and fixes
Leak under the nose
Reposition the cushion with the machine running and even out the straps. If it still leaks, try another cushion size; the frame takes all three.
Red marks
Loosen the straps; a nasal cushion seals with air pressure, not strap tension. Tell your provider about any skin breakdown.
Dry mouth
Air escaping through your mouth. Try a chinstrap, or ask your clinician about a full face mask.
Hose tugging the mask
Let the swivel turn freely and route the hose over the headboard or with a hose holder so it does not pull the cushion.
Cleaning and care
After each use, take the mask apart, wash the parts (not the headgear) by gently rubbing them for up to 10 minutes in warm water (about 86 F) with mild, unscented dish soap, clean the vent with a soft brush, rinse with drinking quality water and air dry out of direct sunlight.
Once a week, hand wash the headgear the same way, rinse and air dry. React Health says not to use anything with bleach, chlorine, alcohol, aromatics, moisturizers, antibacterial agents or scented oils, and to keep all parts out of direct sunlight and heat.
How often to replace parts
- Mask
- 1 every 3 months (A7034)
- Cushion
- Up to 2 per month (Medicare A7032), typical guidance; your plan may differ
- Headgear
- 1 every 6 months (A7035)
Supplies that can help
A soft strap that supports the jaw to help keep the mouth closed, used mainly with nasal and nasal pillow masks to reduce mouth leaks.
A stand or arm that lifts the tubing above the bed so it does not drag on the mask when you turn over.
Pre-moistened wipes for a quick daily wipe of the mask cushion, mostly to remove skin oils between proper washes.
Plain, mild dish soap is the cleaning agent most CPAP makers and the FDA point to for masks, tubing and humidifier chambers.

Frequently asked questions
Is the Rio II Nasal Mask the same as the Rio II nasal pillows mask?
No. They are separate products. The Rio II Nasal Mask has an under-the-nose cushion, while the Rio II Nasal Pillows Mask has pillows that sit in the nostrils.
Does the Rio II Nasal Mask have magnets?
React Health says its masks use easy release clips, not magnets, and that this is common to all React Health masks.
What sizes does the Rio II Nasal Mask come in?
Small, Medium and Large cushions, plus an all size kit. The modular frame fits all three cushion sizes.
How quiet is the Rio II Nasal Mask?
React Health says the circular dispersion vent brings noise as low as 20 dB. That is the manufacturer's figure, not an independent measurement.
