Transcend 365 miniCPAP
The Transcend 365 is a small CPAP from Somnetics with a built-in heated humidifier that sits underneath the machine and is held on by magnets. It came in a fixed-pressure version and an Auto version, both working between 4 and 20 cm H2O, and it can run from the wall or from the optional P10 battery. The humidifier uses a wick and a heater cartridge instead of a large water tub, so it needs only about 195 mL of distilled water.
Sleep Review reported the 365 as newly available on November 8, 2019. As of October 2026 the machines section of the Transcend store (mytranscend.com) lists only the Transcend Micro and Micro 510, so the 365 is no longer sold there. We did not find a dated discontinuation notice from Somnetics. The company still posts the 365 user manual and lists a 2 year warranty for it, and supplies such as filters and wicks are ordered through Transcend customer service at 877-621-9626.

Strengths and trade-offs
Strengths
- A heated humidifier is built in, so you do not have to pack a separate humidifier to travel with humidity.
- At 1.7 lb including the humidifier, it is light enough for a carry-on bag.
- The P10 battery is rated 93.6 Wh, which is under the FAA 100 Wh limit for spare lithium ion batteries carried in the cabin.
- Ramp, EZEX and humidity can be changed from the on-screen patient menu without a computer.
- The power supply accepts 100 to 240 V AC, so it works on US and most foreign outlets with the right plug cord.
Trade-offs
- It is no longer sold on the Transcend store, so new units and parts may be hard to find.
- The humidifier requires distilled (or Transcend-filtered) water; Somnetics says other water damages the heater and can void the heater warranty.
- The reservoir holds about 195 mL, much less than a standard home CPAP water tub.
- There is no SD card; to see detailed data you need the Transcend desktop software over USB or the separately sold Bluetooth module.
- The water reservoir and heating element carry only a 6 month warranty, against 2 years for the machine.
- No heated tubing option is described in the manual, so rainout control depends on the humidity level and room temperature.
Comfort settings you can change
These are in the patient menu. Your prescribed pressure and therapy mode are set by your clinician and are not covered here.
Humidity level
Sets how much moisture the built-in humidifier adds, from 0 (off) to 10 (most).
How to change it: From the home screen press Down to highlight the humidifier, then press Center. Press Up or Down to pick a level, use Left or Right to highlight the green check mark, and press Center to save. Choose the red X instead to leave without saving.
Ramp pressure
Sets the lower pressure that therapy starts at while you fall asleep. It cannot go above your starting pressure.
How to change it: Press Up and Center to open Settings, then Center for Therapy Settings. Move down to Ramp, press Center, use Up or Down to change the ramp pressure and press Center to save. The quick guide notes ramp only works if your provider has turned it on.
Ramp time
Sets how long the machine takes to climb from ramp pressure to your prescribed pressure, from 0 to 45 minutes in 5 minute steps.
How to change it: In Therapy Settings, on the Ramp screen, press Right to highlight Ramp Duration and press Center. Use Up or Down to change it and press Center to save.
Ramp restart
Drops the pressure back to the ramp level during the night if you wake up and want to fall asleep again at a lower pressure.
How to change it: While therapy is running, press the Down button. Holding Down speeds the ramp up to your prescribed pressure.
EZEX pressure relief
Lowers pressure a little each time you breathe out. 0 is off and 3 gives the most relief.
How to change it: In Therapy Settings use Up or Down to highlight EZEX, press Center, pick 0 to 3 with Up or Down, and press Center to save.
Mask and tube settings
Tells the machine which mask and tube you use so it can perform correctly with them.
How to change it: Open Settings, choose Mask/Tube Settings, highlight the item with Up or Down, press Center, pick the option that matches your equipment and press Center to save.
Drying Mode
Runs the blower for 30 minutes without therapy pressure to dry the humidifier, tube and mask. Do not sleep with it running, because it does not deliver your prescribed pressure.
How to change it: From Standby, hold the Up button for three seconds (current manual). Press Center or hold Up again to stop it early.
Cleaning and care
Every morning
Stop therapy and run Drying Mode so moisture does not sit in the machine, tube and mask. Empty any water you will not use before you pack the machine, because the manual says the reservoir must be completely empty for transport.
Reservoir, weekly
Unplug the machine and lift it off the reservoir. Rinse the reservoir with distilled water once a week, put it back on, and run Drying Mode. Refill it with distilled water before your next night.
Somnetics says to replace the reservoir every 6 months or sooner if it leaks or is damaged.
Outside of the machine, weekly
Unplug it and wipe the outside with a dry, lint-free cotton cloth. Do not put the machine or the power supply in water, do not let liquid into any opening, and do not use harsh cleaners.
Air inlet filter, every 6 months
Turn the machine over and flip open the small filter door on one end. Pull out the old filter media, put in a new piece and close the door. Change it sooner if it looks dirty.
Heater wick
Some units have a ceramic wick that does not need replacing if you use distilled water. Others have a soft wick that you change when humidity seems to drop or your nose and throat feel dry.
To change a soft wick, unplug the machine, remove the reservoir, turn the CPAP over, twist the wick cover a half turn counterclockwise and pull it out. Put the new wick in without handling it much and twist the cover a half turn clockwise to lock it.
Mask and tube
Clean your mask and air tube following the instructions from their manufacturers, including which soap to use and how often.
Replacement parts
- Tubing
- Any standard 22 mm CPAP air tube; none is included with the 365. The manual asks for tubes that meet ISO 5367 or ISO 80601-2-74.
- Filters
- Transcend 365 miniCPAP air inlet filter (part 503096). Replace after 6 months of use or sooner if dirty.
- Water chamber
- Transcend 365 miniCPAP water reservoir (part 503093), 195 mL to the fill line. Somnetics says to replace it every 6 months or if it leaks or is damaged. Soft heater wicks are part 503097 (single) or 503111 (six pack).
- Replacement notes
- Parts are ordered through Transcend customer service at 877-621-9626 or your dealer. The P10 battery is part 503098 and the water filter kit is part 503110. Check with your insurer on how often it covers each item.
Troubleshooting
The screen does not light up when you plug it in.
Check that the barrel plug is pushed fully into the machine and the cord into the power supply. Try another outlet and make sure it is not controlled by a wall switch. If you are on the P10 battery, give the machine up to 20 seconds to finish its battery check.
Your nose or throat feels dry.
Check that the reservoir has water and the humidity level is above 0, then try a higher level. If your unit has a soft wick and it no longer seems to draw water, replace it. A dirty air filter can also be a cause, so change the filter media.
The humidifier icon shows a red droplet in the morning.
The humidifier turned itself off overnight. Look at the indicator on the Standby home screen to see why (out of water, tap water, bad water signal or heater problem) and follow the matching fix in the error list.
Water leaks from between the machine and the reservoir.
Unplug, lift the machine off and check that the reservoir seal is seated and not folded over. Make sure you are filling below the max fill line. If the seal has come loose, stop using the humidifier and call your dealer for a replacement.
The pressure feels too strong.
Breathe slowly through your nose with your mouth closed and use ramp if it is turned on. Raising EZEX from the patient menu can make breathing out easier. If the pressure still feels wrong, contact your prescribing clinician or provider; do not try to change the prescribed pressure yourself.
Ramp does nothing when you press Down.
Ramp only works if your provider has enabled it, and the Down button restarts ramp only while therapy is running. Check the ramp settings under Settings, Therapy Settings.
The machine stops during the night.
The manual links this to a poor seal at the mask or tube or to worn parts. Refit your mask, check the tube connection and replace a mask or tube past its service life. If it keeps happening, call your provider's technical service.
The home icon is red.
The machine has logged a fault. Press Center on the home icon to see the number, note it, and clear it with the check mark. Look up the number in the error list and call your supplier if it returns.
No air comes out at all.
Check that you pressed Center to start therapy and are not in Standby. If the screen is on and nothing moves, the manual points to a motor or electronics failure; contact your provider's technical service.
The tap water icon appears even though you use distilled water.
Leftover tap water residue or moisture inside the CPAP can cause it. Rinse the reservoir well with distilled water, run Drying Mode, and unplug and replug the power to reset the indicator.
Travel and power
The 365 weighs 1.7 lb with its humidifier and comes with a travel bag, and its PSA3 power supply accepts 100 to 240 V AC at 50 to 60 Hz, so abroad you only need the right plug cord. Somnetics' manual says not to use the machine above 8,000 ft unless you are in an airplane, which is also the limit of its automatic altitude adjustment. For power away from an outlet, the optional Transcend P10 lithium ion battery is rated 18 V and 93.6 Wh; FAA rules for spare lithium ion batteries allow up to 100 Wh per battery in carry-on bags only, with the terminals protected, so pack it in the cabin. Before you fly, empty the reservoir completely, because the manual warns that transporting it with water can damage the CPAP. We did not find a Somnetics statement about using the 365 during a flight or a DC car adapter for this model, so check with your airline and Transcend customer service before relying on either.
Your sleep data
The 365 has no SD card. On the machine you can open a Compliance Report (press Left, then Center) showing days compliant, average hours, average pressure, AHI and leak for the last day, week, 90 days or all data, plus a Last Session Report (press Right). For full reports you connect the machine's mini USB port to a computer running the free Transcend desktop software, which shows usage, leak, pressure and AHI, builds a 30 day compliance report and exports a PDF; the software also lets you change ramp and EZEX if your clinician allowed it, while pressure stays password protected. The manual also describes the TranSync cloud and an optional TranSync Bluetooth module, and Transcend's current FAQ says the 365 needs that separately sold Bluetooth dongle to work with its newer MySleepDash app. OSCAR does not list Transcend or Somnetics machines: there is no Transcend loader in the OSCAR source code, so plan on using the Transcend software instead.
Supplies that can help
Distilled water is what most CPAP humidifier chambers are meant to be filled with, because it leaves little or no mineral scale behind.
The air inlet filter keeps dust and lint out of the machine and your airway; it is a cheap part that needs regular replacement.
The plain hose between the machine and the mask; it wears out and should be replaced on a regular schedule.
A battery pack that runs a CPAP machine without mains power, for camping, power outages or long flights.
A padded bag that protects the machine, mask, tubing and power supply in transit.
A soft strap that supports the jaw to help keep the mouth closed, used mainly with nasal and nasal pillow masks to reduce mouth leaks.

Frequently asked questions
Is the Transcend 365 still made?
As of October 2026 the Transcend store lists only the Transcend Micro and Micro 510 as machines, so the 365 is no longer sold there. Can I use tap water in the Transcend 365 humidifier?
No. Somnetics requires distilled water because the wick and heater are damaged by minerals, and using other water can void the heater warranty. The alternative it approves is the Transcend 365 water filter kit, and the machine shows a flashing yellow droplet when it detects tap water.
How do I turn on Drying Mode on the Transcend 365?
After you stop therapy, the current user manual says to hold the Up button for three seconds from Standby; it runs for 30 minutes. An older 2018 quick guide said to hold Up and Down together, so use the method in the guide that came with your unit. Do not sleep with Drying Mode on, because it does not give you your prescribed pressure.
Which battery works with the Transcend 365?
Transcend's battery guide lists the P10 battery as the one compatible with the 365. It is rated 93.6 Wh, takes up to about 7 hours to charge, and should be fully charged before first use without the CPAP connected. Run time depends on your pressure, mask seal and humidifier use.
Can I change my pressure on the Transcend 365?
No. Therapy mode and pressure can only be changed by a clinician with password protected access. You can change ramp, EZEX, humidity and mask and tube settings yourself. If your pressure feels wrong, talk to your prescribing clinician.
Does OSCAR read data from the Transcend 365?
We found no Transcend or Somnetics support in OSCAR, and the OSCAR source code has no loader for these machines. Use the Transcend desktop software over the mini USB cable, or the Bluetooth dongle with Transcend's app, to see your data.
