CPAP Whistling Noise: How to Find and Fix It

A whistle usually means air is escaping somewhere it should not: around the mask cushion, at a loose or cracked tube connection, or around the humidifier chamber and its seal. Work from the mask back to the machine. Whistling that comes from the machine itself with everything sealed is a job for your equipment provider.

Signs

  • High pitched whistle or squeal during breathing
  • Whistle that starts when you roll over
  • Hissing near the water chamber or where the hose joins the machine
  • Air blowing into your eyes along with the noise

Common causes

Mask cushion leak
A small gap between the cushion and your skin makes a whistle. A worn, oily or wrongly sized cushion makes this more likely.
Loose or cracked connection
A tube not fully pushed on, or a split cuff, leaks air with a whistle. Philips lists the tube not fully connected as a cause of a leak or whistling sound from the device.
Humidifier seal missing or not seated
Philips also lists humidifier seals that are not fully seated or missing, and a blocked air inlet, as causes of whistling that is not mask leak.
Exhalation vent
Every CPAP mask has vent holes that let exhaled air out. A soft, steady hiss from the vent is normal. Covering or blocking the vent is not safe.

Fixes to try, easiest first

  1. Find the source with your hand

    With the machine running, run your hand slowly around the mask edge, the mask elbow, each hose connection and the water chamber. A leak feels like a jet of air. Fixing the right spot saves time.

  2. Reseat the mask cushion

    Pull the mask a little away from your face and let it settle back with the air on. Adjust straps evenly. Fisher and Paykel caution against overtightening, which can make leaks worse.

  3. Wash the cushion

    Facial oil can stop a cushion from sealing. ResMed suggests a quick daily wash of the cushion with very mild soap.

  4. Push all connections on firmly

    Reconnect the hose at both ends and check the mask elbow and swivel. Replace a hose with any cracks or tears.

  5. Reseat the water chamber and lid seal

    Remove the chamber and put it back until it clicks. Check that the lid seal is in place and not torn. Make sure the air inlet and filter are not blocked.

  6. Replace worn mask parts

    Cushions and pillows stretch and soften with use. Fisher and Paykel note masks need inspection for wear and tear and parts may need replacing.

  7. Ask your provider to check the machine

    If the whistle comes from inside the machine with everything sealed, contact your home care provider. Do not open the device.

When to call your doctor

Whistling is usually an equipment fix. Talk to your sleep doctor if you cannot get a mask to seal, if your machine reports high leak most nights, or if leak is drying your eyes or waking you, because large leaks can reduce how well therapy works.
