Everyday life with CPAP
Can You Sleep on Your Stomach With CPAP? Tips That Work
Stomach sleeping with CPAP is harder because the pillow presses on the mask, but many people manage it. Sleep Foundation says most stomach sleepers can only use a nasal pillow mask, since other masks press into the face and leak. A thin or CPAP pillow and a top of head hose connection also help.
Signs
- Mask pushed into your face when you lie face down
- Large leaks or air in the eyes
- Neck strain from turning your head to make room for the mask
- Hose kinked under your body or pillow
Common causes
- Mask pressed against the pillow
- Face down, the pillow pushes directly on the mask and breaks the seal.
- Bulky mask style
- Sleep Foundation says other masks tend to press into the face and leak, which is why nasal pillows usually work best.
- Hose pinned under you
- Tubing along the temples can be restricted by the pillow or head position.
Fixes to try, easiest first
Use a thin or CPAP pillow
A thinner pillow, or one with cutouts, gives the mask space. Avoid pillows that force your neck into a strained angle.
Ask about nasal pillows
Nasal pillow masks have the smallest footprint and Sleep Foundation calls them the usual choice for stomach sleepers.
Route the hose over the top of your head
A mask with a top of head tube connection keeps the hose away from your face and pillow. Check the tube is not pinched.
Refit while face down
Lie in your stomach sleeping position with the air on and then adjust the straps.
Use a chinstrap if you mouth breathe
Nasal pillows need your mouth to stay closed. A chinstrap can help if air escapes through your mouth.
Talk to your doctor if you cannot find a mask that seals face down, if your leak or AHI readings are high, or if you wake up with your mask off often. A different mask style may be needed.

Frequently asked questions
What CPAP mask is best for stomach sleepers?
Sleep Foundation says most people can only sleep on their stomach with a nasal pillow mask, which is also less likely to cause pain.
Can I use a full face mask on my stomach?
It is difficult because the mask presses into the pillow and leaks. If you need a full face mask for mouth breathing, ask about compact styles and try a CPAP pillow.
How do I stop neck pain from stomach sleeping with CPAP?
Use a thinner pillow and avoid a mask size or pillow height that forces your head to twist. Sleep Foundation warns against positions that strain the neck.
