CPAP Headaches: Causes and Fixes

Morning headaches on CPAP are often caused by tight straps, sinus irritation from cold dry air, or congestion. Loosening the headgear and improving humidification fix many cases, but headaches that continue need a doctor's review.

Common causes

Tight headgear
Straps that are too tight press on your head and face. ResMed warns against overtightening a mask to stop leaks.
Cold, dry air
ResMed explains cold dry air makes your nose work harder, which can cause sinus headaches and irritation.
Low humidification
ResMed says humidifiers help prevent sinus-type headaches.
Therapy not fully working
If your app shows a high leak or a rising AHI, your apnea may not be fully controlled. ResMed notes mask leak, alcohol and medicines can all push AHI up.

Fixes to try, easiest first

  1. Loosen your straps

    Loosen your headgear until the mask just seals. Strap covers can spread the pressure.

  2. Add humidity and heated tubing

    Warm, moist air is gentler on the sinuses. Raise humidity one step and add heated tubing if your room is cold.

  3. Treat congestion

    Saline spray and humidity can ease sinus pressure.

  4. Check your data

    Look at your AHI and leak in your app. High numbers may mean therapy is not controlling your apnea.

  5. Drink water and limit evening alcohol

    Dehydration and alcohol both affect sleep. NHLBI recommends limiting alcohol.

  6. See your doctor with your clinician

    If headaches continue after these changes, ask your doctor to look at your settings and other causes.

See your doctor for headaches that continue more than a couple of weeks on CPAP. Get emergency care for a sudden, severe headache, a headache with confusion, weakness, vision changes or a stiff neck.
