Buying a CPAP Without Insurance (Cash Pay)
You can buy a CPAP without insurance from a retailer, but you still need a valid prescription because CPAP machines and masks are prescription devices in the US. Check that the machine is new or properly refurbished, comes with a warranty, and is not an unremediated recalled Philips device.
You still need a prescription
US CPAP manuals carry the caution that federal law restricts the device to use by or on the order of a physician, and FDA cleared mask documents describe masks as prescription devices too. A legitimate seller will ask for your prescription.
If you have not been diagnosed yet, you will need a sleep test first. Some retailers and telehealth services offer home sleep tests with a clinician review that can lead to a prescription.
What to compare
Look at the machine type (fixed CPAP or auto), whether a humidifier and heated tube are included, mask included or not, the warranty length and who services it, whether data is available in an app or on an SD card for your clinician, and return policies for masks.
Prices change often and vary between sellers, so compare the total for machine, humidifier, tube, mask and filters, not just the headline number.
Used machines
Buying used is risky. You cannot easily verify hours, damage, or whether it is a recalled Philips device that was never remediated. A used machine may also still be tied to someone else's prescription and settings.
If you consider used equipment, ask your clinician first. Never use a secondhand mask, tube or water chamber.
Settings on a cash machine
A cash bought machine still needs your prescribed settings. Ask the retailer or your clinician to set your pressure or range before you start. Do not set therapy pressure from forums or online calculators.
Comfort settings like ramp, humidity and exhale relief are yours to adjust.
Keep paperwork for tax accounts
Save the receipt and your prescription. IRS Publication 502 counts the cost of equipment, supplies and diagnostic devices needed to treat disease as medical expenses, which is how HSAs and FSAs usually treat CPAP. Confirm with your plan administrator.
